Making Healthy Eating and Active Living Opportunities More Inclusive of All People

You are invited to participate in a survey about food and physical activity.  It will take about 5 to 10 minutes to complete. The University of South Carolina Arnold School of Public Health and community partners are working to make sure there are easily accessible places to get healthy food in Columbia, SC.  You might not benefit directly from taking this survey, but you will be able to help guide the ways this group will work to make healthy eating and physical activity more accessible in the community.  The students Arnold School of Public Health might report on the results of this survey in presentations or papers, but we are not collecting any identifying information from you to include.  Taking this survey is your choice.  You do not have to take this survey or answer any questions you do not want to answer.  This project is supported by the National Center for Health, Physical Activity, and Disability (NCHPAD) with funding from the Centers for Diseases Control and Prevention (CDC).  If you have any questions, please contact Carrie Draper, MSW at draper [at]   or call our office at 803-252-2198 if you are interested in taking the survey and would like assistance.
